1.I hereby concur with the COCA-I Gold Ribbon Self-Assessment Guidelines as set forth by the Children’s Oncology Camping Association, International. (Download or view the Self-Assessment Guidelines here.)
2.I hereby confirm the camp I represent (my camp) is operating under the applicable federal, state, and local laws, codes and regulations, and that all required permits and licenses have been obtained. The compliance with legal requirements of the jurisdictions within which a camp is located is the responsibility of the camp; enforcement is the responsibility of the appropriate government official. Where the COCA-I Best Practice Operation Standards require higher performance than the legal requirements, a camp must comply, and maintain compliance, with the COCA-I Best Practice Operation Standards to remain as a COCA-I Gold Ribbon Status camp.
3.I affirm that my camp continues to meet the guidelines as in the current COCA-I Gold Ribbon Self-Assessment Guidelines.
4.Upon being awarded COCA-I Gold Ribbon Self-Assessment Status, my camp will receive from COCA-I a limited, terminable, non-exclusive license to use the COCA-I Gold Ribbon Camp logo for my camp. I acknowledge that my camp's ability to use the COCA-I Gold Ribbon Camp logo shall automatically and immediately terminate if my camp ceases to be eligible for COCA-I Gold Ribbon Camp Status.
5.I understand my camp's right to use the COCA-I Gold Ribbon Camp logo or any other COCA-I trademark is conditioned upon my camp being a member in good standing of the Children’s Oncology Camping Association, International, my camp's completion and delivery to COCA-I of a signed Statement of Compliance, and COCA- I’s confirmation that Gold Ribbon Camp Status has been earned by my camp. I further understand that my camp may not display or use the COCA-I Gold Ribbon Camp logo or any other of COCA-I’s trademarks if:
a)My camp has not paid the necessary fees for COCA-I membership status;
b)A legal representative of the camp has not signed and returned this Statement of Compliance; COCA-I Gold Ribbon Camp Status has never been earned or has been removed for any reason; or
c)Upon notice from COCA-I, my camp fails to promptly cease any use of the COCA-I Gold Ribbon Camp logo or any other COCA-I mark.
d)My Camp acknowledges that all use of the COCA-I logos or marks inure to the benefit of COCA-I.
I have reviewed COCA-I’s Gold Ribbon Self-Assessment Guidelines and I do hereby affirm that my camp meets the requirements established in the Statement of Compliance above. I further affirm that my camp adheres to all of the current applicable guidelines for COCA-I Gold Ribbon Camp Status under which it was last awarded. On behalf of my camp, I wish to continue the COCA-I Gold Ribbon Camp Status.
I understand and agree, on behalf of my camp, that COCA-I Gold Ribbon Camp Status is subject to my camp's continued compliance with the requirements of this Statement of Compliance.
I represent and warrant that I have the necessary authority to execute and agree to all of the terms of this Statement of Compliance on behalf of my camp.
To maintain ongoing COCA-I Gold Ribbon Camp Status, I attest with my signature below, that another Camp representative (Staff or Board Members) and the Medical Director of the applicant camp have reviewed and agree to all the terms of this Statement of Compliance.
